TO GET STARTED, PLEASE FOLLOW THE TWO STEPS BELOW

1. Email a copy of the back/front of your insurance card (Anthem/BCBS or Medicaid only) to: Saren@sagespeech.com - You must have this step completed for us to process your intake form.
2. Fill out an intake form at the link below. You will be directed to the SAGE Speech Patient Portal. Once you have created a login, please SKIP the demographic information by clicking SAVE DEMOGRAPHIC INFO and click directly on the GREEN "Complete Intake Form" button. Once your intake forms are complete, we will contact you for scheduling.
